Question Types
1) Choose One - Allow users to choose only one of the given options
2) Choose Many - Allow users to choose as many as needed
3) Free Form - Allow users to type a descriptive answer
4) Ranking - Allow users to rank the given answer options
5) Scale - Allow users to rate the answer along the given scale. You need to specify the scale range as shown under on the Edit question page.
You can either setup the question so that it includes the scale definition or add the labels to the scale.
Note: Optional question shows the 'N/A' option, but the responses to this option are excluded for average calculations in the summary reports.
"Add Answer" for each topic/criteria that should be rated.
6) Text - Allow users to provide the answer in a text box.
7) Yes/NO - Allow users to give only an Yes or No answer
8) Yes/No/Why - Allow users to select either Yes or No and provide an explanation for their choice. Completing the why field is optional.
9) Yes/No/Why No - Allow the users to select either Yes or No and provide an explanation on selecting 'No'.
10) Yes/No/Why Yes - Allow the users to select either Yes or No and provide an explanation on selecting 'Yes'
11) Select One box - Allow the users to select one of the options from the drop down menu
12) True/False - Allow the users to select either 'True' or 'False'
13) Date - Allows users to pick a date
14) Time - Allows users to pick a date
15) Integer - Use this question type when you want users to enter numbers in the field like IDs where fractions are not allowed.
16) Real Number - Allows users to enter Real numbers like dollar amounts with or without fraction values. Dollar symbols or commas are not accepted.
17) Confirmation Data Grid - Specialized format used to seek users' confirmation. Commonly used in the case of Disclosures and COI Resolution. For this type of question, each answer added (highlighted in yellow) will be treated as a question on its own. you can select the the question type(Free form, Choose one, Choose many etc.,) for each answer. If there is a Lookup list, you can even select that look up here. All the orange highlighted parts are a part of the question type. Users can add as many relationships as needed.
18) Data Grid - Data Grid is similar to Confirmation Data Grid, but without the confirmation part. Commonly used in the case of Disclosures and COI Resolution. User can enter as many relationships as needed by clicking on 'Add Another' button.
Other important fields
Name
The name of the form question. Tip: When you generate reports, this name will become the title of a column in your Excel Spreadsheet.
Question
The text that asks or explains the question. For longer text or a header, you can use the 'Intro Text' field.
Intro Text
For longer or specially formatted instructions, you can create an 'into text' block by selecting 'add text'. Or, if you've already created on, you can select the intro text block from the dropdown. This is especially helpful if your instructions need to contain images or tables.
Display Order
This number controls in what order the questions appear on the form.
Max Text Size
The maximum number of Characters for the response of this question. set to 0 for unlimited.
Scale
If the Question Type = Scale, use this field to set your scale size.
Why Question
For Question Types with 'Why', this is the instruction text that prompts for an explanation. Otherwise, system will display 'why'.

Note
Text that displays at the bottom of the question, typically in a smaller font.
Narrative
Text displays on pre/post test form types only. If there is text to show for the evaluations or other non-question by question forms, added as intro text or added directly to the question, if appropriate.
Required
Any form question marked 'required' must be completed before the form is marked 'complete'.Note: it's important to mark any weighted questions as required.
Special Character Option
When set to 'Yes' on 'FreeForm' Question Type, there will be a special character dropdown box to help people add unique characters to the response. This would be helpful for questions where the responses may contain technical or mathematical information.
Admin Use
When set to 'Yes', this question will only be viewable and answerable by administrators. This is helpful for when administrators need to add notes or feedback to forms, that they can search and run reports against.
Submitter Feedback
If the form is used for Submission Review, marking as 'Yes' will allow the Submitters to see the responses to this question.
Public Use
Most commonly used with Speaker Forms, and Abstract Submission Forms. Sometimes also used with CE Post Test Forms and Information Request Forms.
Speaker Use Only
On a CE Eval or Speaker form, this question will be repeated for every speaker assigned. See related article on how to set up Session/Speaker Evaluations used for continuing education.
Status
If you'd like to remove a question from a form, you can mark it as 'Inactive'. It will still show on the form with a strikethrough.
