Go to Settings > Lookups
Under the 'Administrative Use Only' section, click on the 'Form Types' link
Click on the Form Type you want to edit
Update the 'Name' field and click 'Save'
NOTE: if you do not need certain Form Types for a project, you can change the 'Status' field to 'Inactive'
