Here are the different kind of forms supported in HighMarks CE System.

Application Submission: Forms uses on abstracts and Applications. These are completed as apart of Application Submission process.

Activity Accreditation Info: Forms to collect accreditation info forms at the activity level. For example: ACEP/NABP form to collect the UAN, Activity type and Topic Designator

Activity Overall Evaluation: Form displayed in the Attendee 'My Credits' area for each Activity to collect overall activity evaluation responses.

Anonymous Event Evaluation: Anonymous Event Eval is used for event specific evaluation surveys that are collected anonymously. However, the system only supports one of these forms per site. When used this form will be automatically included in the live session claiming flow.

Anonymous Surveys: Anonymous Survey forms are used to collect form responses without requiring the person to be logged in or without linking the responses to any session or other entity. Sites can have as many Anonymous Survey forms.

Assessment Posttest: This is an activity level post-test. It is not linked to a specific session. It is mainly used to assess the knowledge gained after an activity. It doesn't have any affect on the credit claiming process.

Assessment Pretest: This is an activity level pre-test. It is not linked to a specific session. It is mainly used to assess the knowledge gained before an activity. It doesn't have any affect on the credit claiming process.

Attendee Evaluation: Forms used by Speakers to evaluate attendees.

Attendee Registration Demographic: Form designed for use in the registration flow.

Attendee Request form: It allows attendees to request or submit information. The responses are linked to the attendee signed in or create a new profile if the person is not signed in. We should use this type of request if you'll need the person to come back to the system to do something else later or if you are then going to assign the person to a session or application. Configuration option -(Attendee Request Request with Contact: where Contact Information can be collected on the form)

Event/Lecture: Lecture is used for event specific data not collected elsewhere.

Exhibitor: Form displayed to Exhibitors in their Ready Room.

Exhibitor Contract: Form used to collect exhibitor contract information. Form is available for the general public to submit their request to participate as an Exhibitor at an Event.

Information Request Forms: Can complete, and will be able to pull responses in form report. This is a good form if we just need to collect the contact information and responses for people, but the people don't need to come back into the system to do anything else. Configuration option - (Information Request with Contact where Contact Information can be collected on the form)

Outcomes Survey: Forms to collect the post activity outcomes survey from the people who attended and claimed credits for an event. Review the below document on how to create and set up these forms.
- Planner/Activity: Forms displayed in the Pre/Post Activity requirement space. These forms can be configured to be displayed to the front end roles that have access to this space or restrict them to be visible only to the Admin.

Pretest: CE Pre-Test/ Pretest is required before viewing or attending an event or session. Review the below document on how to create Pretest forms.

Posttest: CE Post Test/Posttest is required after viewing or attending an event or session. Review the below document on how to create Posttest forms. It has an affect on credit claiming process.

Profile Information: Form displayed in the Attendee/Speaker 'My Account' area that is used to collect person specific information.

Session Evaluation: Form used to evaluate an event and the speakers whom gave a talk in the event. Review the below document on how to create Session evaluations.

Speaker: Forms displayed only in the Speaker Ready Room for the roles assigned to Applications and Activities and Events.

Topic Submission: Forms displayed to either attendees or speakers that are allowed to be submitted more than one time. These can be the initial request forms or Preplanning forms to receive an approval to submit a full application.
